There are really two sets of questions here and we’re actively exploring both of them.
1-What caused the explosion and fire on board Transocean’s deep water horizon rig?
2-Why did Transocean blowout preventer, the key fail safe mechanism, fail to shut in the well and release the rig?
Blow out preventer (BOP) is the emergency shut off valve.
BP -> It’s Transocean’s fault because they are responsible for the BOP.
Transocean -> BP sets the specifications for the BOP and the problem had to do with the cementing or casing of the well, which Halliburton did.
Halliburton -> did their job to the specifications of BP.
One and only video released of the leak, despite pressure from the press and environmentalists.
